• The offer is valid exclusively for the first 15 requests to the Referral Office. Purchases made outside this period will not be eligible for the promotion. 
• This promotion is applicable only to self-paying customers through Visa credit card and does not apply to corporate accounts, insurance claims, or third-party payer transactions.
